This document is an affidavit of Aurna Smith, Superintendent of Flagler Hospital, for the date of the death of Abram Lancaster that occurred on July 20, 1904 caused by cancer of the throat. This
document is of interest because it is not a death certificate which could have been obtained from the
St. Johns Department of Health. Abram Lancaster is buried in the St. Augustine National
Cemetery so the date of his death should not have been a matter of contention. Abram Lancaster was a veteran of the 33rd USCT, Company F.

In the matter of the Claim of Hester Lancaster, widow of Abram Lancaster, Co "F" 33d USC Vol Inf No 848.540.

Personally appeared before me a Notary Public in and for the State of Florida. Aurna Smith aged 56 years a resident of St.Augustine in the County and State aforesaid who being duly sworn
deposes and says in regard to the aforesaid case as follows.

I am the Superintendent of the
Flagler Hospital which is located in the said City of St. Augustine and I am the custodian of the books and records of said Hospital. That according to said records  
Abram Lancaster died in said Hospital on the 20th day of July 1904 of cancer of the throat. I further certify that I am in nowise interested in this cas nor am I concerned in its prosecution.
